1. Select ‘Book Appointment’

2. Select ‘Video’

3. Select ‘myself’ or ‘someone else’ for who you are booking

4. Select an Appointment Type ‘Individual traveller’ or ‘Group’

5. Select a Practitioner

6. Select a date and time from the calendar

7. Sign in or create a new patient profile

8. Complete the online health assessment questionnaire. If you’re booking as part of a group, you’ll receive an email with a link to the questionnaire that can be used multiple times or forwarded to each member of your group to fill out.

9. Submit paper vaccination records to info@travelhealthconnect.ca prior to your appointment, or download the canimmunize app to transfer your records onto an online format. All of your health care providers can then update your app for you with any future vaccinations.

10. One of our travel health clinicians will consult with you virtually on any device (chrome browser is recommended)

Following your consultation, we’ll connect you with a local clinic or pharmacy near you where you can receive your vaccinations and other medications.
